Fees and Insurance

Direct payment is accepted If Dr. Ho is not in-network with your insurance plan or you wish to pay for services privately.

Some insurance plans are accepted if Dr. Ho is in-network with your insurance plan.

A valid credit card must be on file with the practice prior to receiving services (regardless if direct pay or using insurance). American Express, MasterCard, Visa, and Discover are accepted. HSA/FSA cards can generally accepted accepted, as long as they are issued by one of the major card brands (Visa, Discover, Mastercard, or Amex), have a valid zip code, is accepted/verified by the payment processor, and a secondary valid credit card is on file as well. Payment of fees are expected at the time of service.

Initial Consultation

Adults:

$400 | 60 minute appointment

$700 | 120 minutes (split into two 60 minute appointments)

Children/adolescents:

$700 | 120 minutes (split into two 60 minute appointments; 1st appointment in a joint session with parent/child and 2nd appointment is a parent-only session)

Established Patients

Adults/adolescents/children:

$250 | 30 minute follow up appointment

$250 | 30-45 minute appointments for cognitive behavior therapy for insomnia (CBT-I)

Out-of-network reimbursement: If Dr. Ho is not in-network with your health insurance networks, you may seek, at your own discretion, out-of-network reimbursement from your insurance plan. Dr. Ho can provide documentation (a.k.a. “superbill") of services rendered, which you would provide your insurance company with. To help you, Dr. Ho has registered his practice with a software platform called Reimbursify that may assist you with this process. Please note that Medicare does not permit out-of-network claims.

Note: Dr. Ho is not a registered participant of any Medicaid, Medicare, or Medicare Advantage plans, including those affiliated with the networks stated above, and cannot submit any claims to such plans.

Payment responsibilities: You will be charged the copay amount, as determined by your insurance plan, on the date of the appointment

Additional payments as determined by your health insurance plan (eg, coinsurance, deductible payments) may also apply

Eligibility responsibilities: As the health plan member, you are ultimately responsible for verifying if your insurance plan will cover Dr. Ho's services. You can take steps to verify yourself by either calling your insurance company, reviewing the "summary of benefits coverage" that you received from your employer or when you purchased your insurance on the healthcare exchange, and/or visiting your insurance company's website, usually via a client portal. If your health insurance plan later determines it does not cover Dr. Ho's services or Dr. Ho is not in-network with your health insurance plan, you will be directly responsible for payment for services at the fees listed on this website.)

Late cancellation/no-show policy: The scheduled appointment time is reserved specifically for you (no double-booking). Appointments that are cancelled within 48 hours of scheduled time, or if you do not show up at the scheduled time, will be charged a fee of $150.

Professional services requested outside of scheduled appointment times (eg, completing forms, extended telephone calls): $100 per 15-minute interval.
