Please click below and provide your name and email address. You will receive an email with access to a secured form to initiate your request for an initial consultation.

What to Expect

  • 1) Complete Initial Consultation Appointment Request Form. Your request will be reviewed and you will be notified within 3 business days. If you are using your health insurance plan, your health plan information will be verified as a courtesy.

    2) Complete intake/consent form packet, that will be emailed to you, if your request is approved.

    3) Schedule your first initial consultation appointment. Upon completion of the intake/consent form packet, you will be provided a link to conveniently book the appointment. Appointments are generally available within 1-2 weeks after completing forms.

    Please check your email for confirmation when each step is completed. If you have not received the email, double-check your spam/junk folder or contact the office.

  • All initial consultations begin with a 60 minute appointment at Dr. Ho’s office in Addison, TX

    In this initial appointment, we will meet to have a guided and thoughtful discussion. The focus will be to understand your concerns, review your mental health and medical history, and conduct a comprehensive psychiatric diagnostic evaluation.

    Dr. Ho will also share diagnostic impressions, discuss treatment recommendations, and answer questions you may have.

    After the initial 60 minute appointment:

    Adults: an additional 60 minute appointment may be needed to complete all of these items. Dr. Ho will discuss with you during the 1st appointment if this 2nd appointment is necessary.

    Children/adolescents: a second 60 minute parent-only appointment is required to complete the consultation.

    Logistics of the 2nd appointment (if applicable): The 2nd appointment can be held in-person or via telehealth and will be arranged when you meet with Dr. Ho for the first appointment. The 2nd appointment typically can be scheduled the same week of the 1st appointment or the following week(s).

    Establishing care: This consultation is for the purpose of evaluation only and does not establish nor guarantee an ongoing physician-patient relationship with Dr. Ho. At the conclusion of the consultation, Dr. Ho will discuss his recommendations with you and you and Dr. Ho will discuss whether establishing care is appropriate. If so, follow-up appointments will be discussed. If this is not the case, Dr. Ho will provide an appropriate referral.  Medications are not necessarily prescribed during the consultation until the evaluation is completed.

  • Treatment consists of follow-up appointments which are 30 minutes and can be held in-person or via telehealth.

    Dr. Ho will meet with you to discuss how you are doing with the treatment plan with ongoing diagnostic evaluation and education. This may also include both medication management and non-medication strategies to complement your treatment.

    Frequency of follow-up appointments are determined by your needs and medical necessity (ranging from weekly to every 3 months).

  • Patient Portal: Once established, patients can conveniently manage appointment times in the Patient Portal. A secure messaging portal is also available for exchanging communication regarding brief matters (eg, medication refills).

    Collaboration: When necessary and with your consent, Dr. Ho can discuss treatment plans with other members of your treatment team (ie, therapist, primary care clinician).

    Additional Services: Clinical work requested to be completed outside of scheduled appointments will be charged a fee ($100 per 15 minute interval). Examples include completing forms, extended telephone calls/portal messages, etc. It is recommended to make an appointment and have such items completed during the appointment. This will not only avoid these fees but also make this a collaborative process between you and Dr. Ho.

    In-Person Appointments

    Dr. Ho’s office is located in Addison, TX. The first appointment is held in-person. Subsequent appointments are available as in-person appointments.

    Telemedicine Appointments

    After the first in-person appointment is completed, telemedicine appointments are available, through a HIPAA-compliant telemedicine platform. Patients must be located in Texas during these virtual appointments.
